Appointments
Before you call, please have available:
- Your insurance card
- The full name of your referring doctor (if you have been referred)
- The reason you are being referred
To make an appointment for OAKLAND:
To make an appointment for SAN RAMON:
Before your appointment, please collect the following and bring to your appointment:
- Your written referral (if required by your insurance)
- Your insurance card
- A written list of all your medications
- Any x-ray, CT scan, or MRI scan films you have had related to the current problem. We may need to see the actual films to make a diagnosis (although these may be on a CD-ROM). Films can be obtained from the facility where you had the x-ray or scan
- Any available notes from prior neurologists
- Copies of any blood or other tests related to the current problem
- Completed patient intake form, which can be printed ahead of time, or we can mail it to you if you call at least a week before your appointment
- If the visit is for memory problems, the patient MUST be accompanied by a family member or caregiver
