Dear Valued Patient,
We are sorry to announce that due to economic forces beyond our control, Associated Neurology Medical Group will be closing permanently as of February 19, 2018. On February 20, 2018, we will be joining Sutter East Bay Medical Group (SEBMG), a multi-specialty physicians group, affiliated with Sutter East Bay Medical Foundation (SEBMF). SEBMF will operate the business aspects of our medical practice, allowing us to devote 100% of our time to patient care. Our new locations will be at 2850 Telegraph Ave., Suite 110, Berkeley, CA 94705 and 20101 Lake Chabot Rd, Castro Valley, CA 94546.
It has been our privilege to be your physician and we hope to maintain this relationship.
We anticipate that most of the current plans insuring you today will be accepted by SEBMF and have enclosed a list of contracted HMO plans for your information. If you are a PPO patient, we do not anticipate that you will need to make changes to your insurance plan. For insurance questions, please feel free to call SEBMF’s insurance line at 510-204-6707 to verify whether your current insurance is accepted.
If after January 2 you would like to schedule an appointment for one of our new locations (starting February 22), please call 510-204-8140.
Note: You do not need to transfer your medical records if you continue to see Dr. Collins or Dr. Starkey at their new offices at Sutter.
If you do need to have your medical records transferred, you need to mail us the "Authorization to Release Medical Records" form. First, download and print the form. Then fill out the form and mail the completed form to us. There is $35 fee for this service. Click on the button below to access the form:
Your existing medical records will remain in our possession for 7 years.
