Dear Valued Patient:
Over the past several years, I have been a part of Associated Neurology Medical Group. This letter shall provide notice that I will be leaving this group and have made the decision to open a private practice at two new locations effective February 1, 2018.
I will now be seeing patients in Alameda and San Ramon. The Alameda address will be 985 Atlantic Ave #300, Alameda, CA 94501. In San Ramon, the old office at 5401 Norris Canyon Road, Suite 306 is closing effective January 31, 2018. My new office will be located across the parking lot at 5601 Norris Canyon Road, Suite 240, San Ramon, CA 94583. I will continue to administer Botox at both practice locations.

I will maintain all paper medical records from Associated Neurology at my Alameda location.
Unfortunately, for those of you that remain with Affinity and Hill Medical Groups, I am no longer participating. With a copy of your signed release, I will be happy to forward your medical records to your new neurologist.
